Keil Programming/Debugging Failing (st-link verified ok)

Hi, I am having trouble downloading firmware as well as debugging using Keil v5.24. Every time I attempt to download an image or debug, i get a failure that the target cannot be accessed, and the program is shutting down. The debug output from keil is as follows:

Erase Failed!
Internal command error
Internal command error
Error: Flash Download failed - "Cortex-M4"
Flash Load finished at 15:23:40

I have verified that I can program the firmware image using the st-link utility, so i know the programmer and board are fine. Any suggestions on how to resolve this?
