I read about the RealView MDK v4.54 product in the Mazidi ARM book. After seeing a summary on the Keil website, it seems like a tool I would like to have. Is there a fee associated with it ? Wher can I download it please ?

MDK version 4.54 can be downloaded with this URL if you have a valid license: https://www.keil.com/update/sw/mdk4/4.54
If you don't have a license, you can only use the latest version of MDK5 or MDK6 with a community edition license. Please see:https://developer.arm.com/Tools%20and%20Software/Keil%20MDK
No doubt, I'm not understanding what RealView is. The link you directed me to, for the newer versions, doesn't mention RealView. I'm using Keil 5.42. Is RealView built in to the MDK or a separate tool that i need to add to my project or installation ?

The name 'RealView' was used as a brand name for some years for all development tools. This name was just a marketing decision. The development tools are the same. The name originally came from our debugger many years ago. For microcontroller development tools we now continue to use our brand name 'Keil', for example 'Keil MDK', 'Keil Studio' etc.
