At MDK's Flash Download Option configure for 'Erase Full Chip' and retry.

What board is it on?
If it's your own custom board, how have you proved that the board design is correct, and the hardware good?
Before going to a custom board, have you been able to program a chip on a known-working Dev Board or similar?
